Referrals
Insurance Covered Physical Therapy
This is a more traditional way to utilize SMART Physical Therapy. You may see a Doctor for a condition for which he or she feels you could benefit from Physical Therapy. The Physician will give you a piece of paper called a referral. The referral provides information to the Therapist to help direct their evaluation and treatment. The referral you are given can be used at any Physical Therapy office. Some insurance plans do not require you to have a Physician's referral to receive Physical Therapy.
What is Direct Access?
Direct Access allows you to be evaluated and treated by a licensed physical therapist in the State of New Hampshire, without a prescription or referral from a physician.
How can I benefit from Direct Access?
Direct Access provides you with immediate and economical access to physical therapy services. In a study conducted to determine whether direct access to physical therapy services is cost effective, it was found that:*
The total paid claims for physician referral episodes to physical therapists was 123%, or 2.2 times higher than the paid claims for Direct Access episodes. (The total paid claims averaged $2,236 for "physician referral" episodes as compared to $1,004 for Direct Access episodes.)
Physician referral episodes were 65% longer in duration than Direct Access episodes.
Physician referral episodes generated 67% more physical therapy claims and 60% more office visits than Direct Access episodes.
Will my insurance cover my visits?
Although all patients in New Hampshire have direct access to physical therapy care, each health insurance company has its own requirements for coverage. Some health insurance companies require their members to have a referral from a physician for physical therapy services. We recommend that you contact your insurance company to confirm your benefits.
What if I need a physician in addition to physical therapy care?
SMART PT will refer you to a physician if your physical therapist determines that your condition requires a physician's evaluation.
Is it expensive to come without insurance coverage?
SMART Physical Therapy has a very competitive price for cash patients. We charge 100$ an Initial Evaluation and 50$ for a 30 min follow up visit. In some cases a patient may have a very high deductible and choose to pay as a cash patient, especially at the end of the year when they will not meet their deductible in the shortened time period.

Payment
SMART Physical Therapy will submit claims with your primary insurance carrier. During your first visit we will contact your insurance company to verify the specifics of your coverage and payment plan. For the portion of your treatment that your insurance does not cover, we accept payment via cash, check, and credit card. Payment is expected at the time of treatment.
